After 5 years of riding and about 10 well-schooled dressage horses I still can't do half-passes or leg yields when not out on the trails.
So grateful that my horse puts up with my dressage nonsense

I was in a very similar situation recently. I found I was getting much too wound up, and causing myself, and the horse much more anxiety then ever needed in such a small situation.
What worked for me was I took a step back, I tried to veiw it from another persons perspective. (Recording and watching it back helped me) And I started from square one. Not for the horse but for myself. Start by walking for 20 minutes, maybe try to get her to frame in the walk. Work on your position and the simple things that you need to change all while at the walk. Do a few rides like this. Then move to a trot, do the same thing. Work slowly, and if you feel yourself becoming overwhelmed simply go back one step. I found this really helped with confidence. And I found ground work helps a ton. Getting to see a horse move, to see what they can do without our guidenece can show you how little guidence they may need.
And one last thing, have fun while you ride! Donร‚โ€™t beat yourself up every single ride about every single thing. Itร‚โ€™s not good for you or your horse. Mix it up. Have a bareback ride, try western, try riding in a halter, going for a hack, fun stuff like that will really help you relax while riding.

Bring it back down to not trying to accomplish anything while riding. Just get out there and do something new. Do some groundwork, when you feel like a team it's easier to get things done. Don't fret, even though it sucks to be in the position. You're lucky to get out and ride. You'll get past these feelings soon enough and gain more confidence!

Dont think of what you COULD be or where you WERE or what you SHOULD be. Get out and have fun!

Riding is something that not everyone has experienced, and while us equestrians have a tendency to get worked up and forget about why we’re even riding, we sometimes forget to have fun. Instead of working yourself up (and your horse in that case too), think about the experience you are gaining while riding. While it feels great to dismount and say “Wow! What a great ride!”, it also feels great to think about how one day you will achieve your goal.

So feed the positive dog and don’t forget to have fun!!! Try some new things, some new drills!! Even if you have to pop some earbuds in and listen to your favorite song over and over again to prevent yourself from getting worked up, then do it!!!

I also have some fun games to play on horseback that my friends and I enjoy:
-Egg & spoon:
Take a small ball and place it on a spoon. See how long you can keep the “egg” on the spoon. It’s especially fun and challenging at the canter!!

-Water gun race:
You need at least two people to do this one, and I do not recommend doing this with a horse that spooks a lot. Make some targets about the size of a basketball (even paper plates work) and pin it to the back of your shirt. Try to hit your opponents target with your gun!!

-Batton toss
This game requires two people, but it is especially fun with multiple pairs of two. Once again, I do not recommend trying this game with a horse that spooks a lot. Riding side by side with a partner. toss a small (catchable) item to eachother. First team to drop their item is out.
We use battons. but bean bags also work pretty well.

i would take a break from training and just have some fun, go on hacks into the country and just do some pleasure riding for a while, also do some groundwork so you two can bond again fully. Dont take it to hard and dont be so hard on yourself, every rider will get to this point and when they do they need to slow down and take a little break and just enjoy.

After a little break and just enjoying your girl, slowly pick up the training again first to the things she and you fully know. Once you get to the point where you think its as far as you can go you will need to get a new trainer in to train you both. Also a Solution could be to get someone more experienced to ride your horse once or twice a week, we here in germany call it correction riding, basicly the person will correct the horse back to the normal stage and give you tips to improve yourself, not sure if you have something like that where you are. Doing this does not mean you are a failure atall it just means that you need a little help to stay on the right path. There is no shame atall in not doing something right or not doing something perfect.
No one ever is perfect!

Hope this maybe will help you a little but please dont beat yourself up even a olympic rider will have this blockage at some point.

This is exactly what helped me! It is really helpful too that this trainer has an apprentice type program so sometimes I will get a lesson from someone else who has a different perspective and helps me focus on new things. I have found it is all about shaking it up and not getting stuck in the same old rhythm.
