Here's something I've never seen happen (look at breeding history):

This happens when the mare is bred after midnight game time but before rollover at 2:30 AM game time, then again after rollover.

This one is a true bug. The foals were born on Feb 4th, and the mare had already had a January foal so it was not a rollover situation.
