Do you need to credit purchased Photoshop actions and filters, or is it just brushes?

Is I use an app to edit the photo that I took then uploaded it using Deviant, how would I credit it?

When it is I that made the picture, do I put the (C) and stable name on the picture or somewhere else?

Yes, you can do (c) Your stable name or ID number. :)

You don't need to credit the uploading service, you just need to credit what is IN the image. If you took the photo yourself, you'd credit (c) Your stable name or ID number.

How exactly do I credit? The image I have for my icon has the artist's watermark on it as it was a paid commission, and I've credited them in my shoutbox.

I usually use their username and @pixabay

You cannot credit them in your shoutbox, this was repeated multiple times. Yu can only out the credits ON the image.
