What do these numbers mean and how do I use them

Those are weekly training records. So Wk5:212122 means that in the horse's 5th week of training those were how many bars were up in each discipline. This type of tracking helps you find a horse's strengths and weaknesses which allows you to pair them when breeding to a horse that is strong where your horse is week.
With the numbers you posted, that horse is stronger in Movement, Heart, Speed, and Strength. It is pretty low in Intelligence and very low in Scope. At this point in the game, skills up week 4 are strong, week 5 is about average (ideally you want all up this week), and week 6 are relatively okay but weaker.
You want your week 4 training to match your week 8. If your week 4 is 121122, your week 8 should be 232233. Your week 10 should look like 333333 if it is a quality horse (there are exceptions).
